Parkside Surgery and the CLARE project are working in partnership to pilot a GP social prescribing model for older people living in North Belfast. The pilot will enable people to live independently and stay socially connected within their communities by providing assistance to connect to existing services, support to attend health care appointments and offer volunteer befriending in the home and on social outings.
